A summary of the specific skills, knowledge and behaviors of this position are: The Licensed Practical Nurse (LPN), under the direction of the RN and within the scope of LPN practice, shall provide care for patients, residents and clients including, but not limited to: Implementing the plan of care; Performing nursing procedures; Participating in and facilitating patient, resident and client and family teaching; Communicating pertinent information to other members of the health care team; Maintaining standards for professional practice by demonstrating competent age appropriate clinical skills.
Education - Completion of an approved practical nursing education program with an OBN approved medication administration course.
Experience - Previous LTC experience preferred.
License/Certification - Current applicable state practical nurse license. CPR proficiency. IV therapy card verifying successful completion of state approved IV therapy course for LPNs. Evidence of successful completion of an approved dysrhythmia identification course (if applying for telemetry areas).
Skills - Work under stressful conditions and/or irregular hours. Ability to handle stress and changes in work environment and routine in appropriate manner. Mental alertness and critical thinking skills in stressful situations appropriate to licensure/certification. Positive attitude toward health care members and diverse patient/resident population.
Risk Category - Cat I: Exposure to blood/body fluids/tissue.
Working Conditions - Is subject to working beyond normal working hours, on weekends, and holidays, and in other positions temporarily, when necessary. Working throughout the facility moving around intermittently throughout the shift. Frequent interruptions.
Physical Demands - Walking and standing the majority of the day; able to lift or move 50 pounds; near visual and hearing acuity (assistive devices may be appropriate); motor coordination and manual dexterity.
Personal Protective Equipment Requirements - As required in the MSDS Safety Manual.
JOB SPECIFIC ACCOUNTABILITIES

Under the direction of the RN, contributes to completion of comprehensive assessment of patient/resident/ client needs. Through data collection, identifies significant changes in patient resident client condition and reports them to the RN. Obtains complete nursing history from patient/resident/ client and family. Completes ongoing data collection as determined by patient/resident/ client's condition. Obtains information to contribute to physical, psychosocial and spiritual database. Collects data to contribute to the total assessment of the patient/resident/ client. Routinely reviews medical records (i.e., care plan, multi-disciplinary progress notes, physicians' orders, medications, diagnostic test results, etc.). Utilizes observation, interview, data history, and knowledge of growth and development, information from other caregivers and disciplines in order to obtain a comprehensive database.
Participates in planning and implementing care based on patient/resident/ client data, as well as a sound knowledge base. Follows an individualized plan of care. Adapts to the changing needs of the patient/resident/client and the unit department; organization by incorporating the following skills: time management, organization, and prioritization. In collaboration with RN, physician, family and other health care professionals, gathers data to establish discharge planning needs upon admission. In collaboration with a RN, gives discharge instructions and assures the patient/ resident/client family or significant other indicates understanding. Maintains the standard of nursing care and functions within the policies and procedures. Performs care and documents in medical record the care given, responses and the effectiveness on interventions. Demonstrates ability to organize, prioritize and adapt to the changing needs of the patient/resident/client. Understands legal responsibilities and adheres to legal requirements within the scope of LPN practice. Demonstrates ability to address physical, psychosocial and spiritual needs of patient/ resident/client. Acts as an advocate in all aspects of care including legal/ethical issues. Participates in advanced directives process. Participates in quality and performance improvement initiatives. Demonstrates appropriate procedures in receiving/transferring patient/ resident/client. Participates in limited IV therapy.
Under the direction of the RN, continuously recognizes and evaluates results of care, significance of changing patient/resident/client condition. In collaboration with a RN, reprioritizes care based on changes in information and evaluation of interventions. Documents the patient/ resident/client's progress towards meeting desired outcomes. Gathers information related to the discharge plan and documents achieved goals. Assists in identifying needs for additional intervention involving community resources. Demonstrates appropriate interpersonal and communication skills.
Provides patients/residents/clients with a safe and supportive environment based on growth and developmental needs. Conveys relevant information to other members of the health care team. Demonstrates understanding of emergency procedures and responses. Demonstrates required competencies. Practices principles of infection control, body mechanics, hazardous waste management, electrical safety, patient/resident/client safety. Maintains work area, patients/residents/clients’ rooms, and equipment in clean orderly safe manner, ensuring availability and proper functioning of supplies and equipment. Demonstrates safe administration of medications. Follows policy on patient/resident/client abuse.
Days - 7 AM - 7:30 PM
Nights - 7 :30 PM - 7 AM
(Rotating Weekend and Holiday Rotation Required)
