Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and

exemplifying the service excellence goals of care, concern, respect and cooperation. As a contributing

member of a health care team, functions in the role of patient advocate by being in the position of initial

contact for patient-related issues. Recognizes and consistently applies knowledge of Loyola's 14 Forces

of Magnetism.

PatientAssessment-Functionsintheroleofpatientadvocateby beinginthepositionofinitialcontactforpatient-relatedissues. Clarifiesandimplementsphysicianordersbyadministering medicationsandperforming/orderingtreatmentsandtestingas prescribedbythephysician(MagnetForce#12).Recognizes symptomsandestablishesprioritiesconsistentwithpatient'scondition (MagnetForce#9).Documentspatient'sphysicalandemotionalstatus andaddstoplanofcareinordertoaddresspatientneeds appropriately(MagnetForce#'s5,6). 2 25% Yes PatientCare-Reviewsandevaluatesplanofcare,obtainsinformation frompatientand/orfamily,andaddstoplanofcareasappropriate (MagnetForce#'s2,6).Collaborateswiththemultipdisciplinaryteam, thepatient,andfamilytopromoteefficientpatientcare(MagnetForce #'s10,11,13).Providesforthephysical,culturalandpsychosocial needsofthepatientandfamily.Integratesevidence-basedpractice andprofessionalstandardswheneverpossible.Servesasachampion forpatientsafetybycontinuallyobservingandmonitoringtheclinical environmentandassuringthatsafetygoalsandstandardsare consideredwhenprovidingcareinthepracticesetting. 3 10% Yes Communication-Performsrequireddocumentationandcommunicates inacompetentandeffectivemannertomeetpatientand/orfamily needsandregulatoryrequirements.Protectsconfidentialityofpatient's PersonalHealthInformation(PHI)(MagnetForce#'s3,8,13). 4 25% Yes WorkFlow-Assistswiththedailyoversightofotherclinicalstaff(other nurseandnon-nursestaffmembers)whendirectedbythemanagerto maximizeworkflowinthepractice.Providesfeedbacktothemanager whenrequestedregardingwork-relatedskillsofnon-licensed personnel.Servesasamentorandpreceptorfornewstaffandasa constantresourcetonon-licensedornon-nursepersonnel.Remains flexiblewithworkassignmentsthroughouttheclinicalsitetoassure appropriatecoveragetomeetstaffingandpatientcareneeds. 5 5% Yes QualityImprovement-Utilizestimeandresourceseffectivelyto promotequalityandcostoutcomes.Participatesinsite-specificand divisionqualityprojectsandinitiatives.Servesasaclinicalexpertby reportingpatientrelatedissuesandconcernsdirectlytothemanager andmedicaldirector.Assumesanactiveroleinmaintainingsite preparednessandcompliancewithguidelinesandstandardsas definedbyregulatoryagencies(ie.TheJointCommission,Illinois DepartmentofPublicHealth)(MagnetForce#'s1,7). 6 5% Yes Billing&FacilityCharges-Assuresaccuracyofpatientchargesand directspatientsappropriatelywithbilling-relatedissuesor pre-certificationneeds. 7 5% Yes Triage-Handlesnurse-directedtelephonecallsfrompatientsand seeksappropriateinputanddirectionfromthephysicianorfunctions fromsignedprotocolsifapplicable.Documentsdispositionofcallwith theappropriateinterventions. 8 5% Yes ProfessionalDevelopmentActivity-Demonstratesaccountabilityfor professionalgrowthandjobperformance.(MagnetForce#'s4,14).
