Edward-Elmhurst Health Advance Practice Nurse, EHV in Naperville, Illinois
The APN Fellow provides advanced care for patients in the emergency department, medical floors and behavioral health settings. In collaboration with the physician and other health care team members, the APN Fellow plans and coordinates care to achieve optimal patient outcomes. During the fellowship program which may last up to four months, the APN Fellow shadows established providers and may implement individualized treatment plans, evaluates outcomes, modifies treatment plans as needed, coordinates care for complex patients, provides patient education, and facilitates discharge planning for patients.
PRINCIPAL DUTIES AND RESPONSIBILITIES: (The following duties and responsibilities are all essential job functions, as defined by the ADA, except those that begin with the word "May.")
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
Shadows established Psychiatrist, Nurse Practitioner or Physician Assistant.
Rotates through various EEH and LOH hospital settings to gain additional skills and practice knowledge
Attends weekly supervision meetings to review cases
Attends monthly education courses and lectures to enhance knowledge base
Not allowed to prescribe any pharmacological agents and/or treatments based on clinical indicators
The fellowship role will last approximately four months
The individual may be moved to an APN title once they have achieved all the following:
Passed National Certification in Psychiatry exam
Current State of Illinois APN license
A collaborative practice agreement
Current State of Illinois Controlled Substance license
Current DEA license
Successful completion of fellowship
