POSITION SUMMARY: The incumbent in this position is a Licensed Practical Nurse responsible for identifying basic patient needs and care through assessment, planning, and evaluating the patient's response to care and progress toward patient outcomes, the family or those significant to the family are considered as part of the assessment, planning, intervention, and evaluation efforts. The nursing care provided is on the basis of actual needs and treatment, meets hospital and Division of Patient Care Services Standards of practice, and reflects the philosophy of the department. POSITION DUTIES:

  • Gather pertinent patient information for provider or registered nurse for appropriate patient disposition

  • Administer medications/immunizations

  • Draw labs if needed

  • Ensure that abnormal labs and tests are addressed by provider team in a timely manner

  • Resolve patient clinical inquiries/phone triage messages, escalate to RN or provider as necessary

  • Ensure that prescription refill requests are processed timely

  • Perform quality audits as requested

  • Serve as back up for all MA functions

  • Release future orders when applicable.

  • Work as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.

  • Collect samples, such as blood, urine, or sputum from patients, and perform routine laboratory tests on samples.

  • Prepare patients for examinations, tests, or treatments and explain procedures.

  • Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.

  • Help patients with dressing, moving on or off exam table, or standing and walking.

  • Set up equipment and prepare medical treatment rooms.

  • Monitors Epic Pools

  • HOS Scoring

  • EMR experience preferred

    JOB SPECIFICATIONS: Education: Required: Graduate of an accredited LPN program/school Experience: Preferred: Experience in ambulatory care.
