Vitra Health Admissions Registered Nurse - RN Case Manager in Quincy, Massachusetts

Essential Functions

  • Knowledge of regulations for all of Vitra's Programs

  • Perform chart audits throughout the admissions - intake process to ensure program compliance

  • Review all medical documentation to ensure it meets or exceeds requirements for insurance authorization

  • Conduct in-person assessments of potential clients to determine eligibility for the program and determine level when appropriate

  • Evaluate AFC/GAFC caregivers in conjunction with the Care Manager

  • Complete required assessments and documentation including the Minimum Data Sheet, Multidisciplinary Care Plan, and other documentation accurately and in a timely fashion

  • Work collaboratively with the interdisciplinary team to transition clients post-admission

  • Work collaboratively with the Intake Team to ensure that clients are admitted to the program(s) in a timely manner

  • Communicate with clients, caregivers, community partners, as needed

  • Maintain client charts and document efforts in our process management system throughout the intake process

  • Other duties as assigned

Requirements:

  • Registered Nurse - RN Massachusetts license required

  • Case management and/or nursing education experience strongly preferred

  • Experience with Minimum Data Sheet (MDS) and MassHealth Programs preferred

  • Minimum of 1 year experience in an acute or home health care setting or comparable work experience recommended

  • Past experience working with behavioral health population is a plus

  • Fluency in both English & Spanish preferred

  • Strong communication, writing, organizational, and time management skills

  • Solution oriented with the ability to multitask
