Summary The primary purpose of the Nursing Assistant (NA) position is to assist licensed nursing staff in the treatment and care of patients/Veterans as a member of the nursing care team in the Primary Care Clinics, Outpatient Nursing Service, at the Mann-Grandstaff VA Medical Center. Nursing Assistant assignments typically involve complex nursing needs which can vary within a range of predictable to unpredictable requirements. This is the full performance level of the position. Responsibilities Duties will vary based on grade. At the GS-5 level, duties will include, but are not limited to: Assist in the full range of nursing care to patients/Veterans with physical and/or behavioral problems in the Primary Care Clinics, outpatient setting, under the direction of a RN and or LPN/LVN. Assists health care providers in performing patient care services and duties pertaining to the smooth running of the Primary Care Team. Performs clinical and clerical duties in order to provide a "seamless" process for patients. Perform treatment procedures such as application of hot and cold packs, irrigation of wounds, and changing of sterile dressings. Prepare equipment and assumes responsibility for cleaning, decontamination, and sterilization. Perform various procedures as assigned for patients who are medically ill under direct supervision of a Registered Nurse. Perform EKGs, ear irrigations, standby as needed for gender specific exams, and clinic flow time studies accurately and in a timely manner to assist in keeping the clinic running efficiently. Recognize and react to emergent patient/resident care situations and intervenes while waiting for assistance. For example, recognize the need for basic life support (BLS), control bleeding and assist with behavior crisis, etc. Participate in code emergency situations by assisting staff. Order lab work and x-rays as directed by provider. Support patient teaching based on evidence-based, general information covered with the patient previously by either the doctor or nurse. Assist other patients and/or family members in the area and provide emotional support. Independently collects a wide range of specimens including urine, throat and wound cultures. Assist the provider in the collection and transport of specimens. Assist the provider in the collection and preparation of tissue biopsies. Transport and properly document/sign in tissue biopsy specimens. Instruct the patients on his/her role in the various procedures, such as biopsies, covering information previously supplied by the provider or nurse. Instruct the patient on follow-up routine care and appointments. Perform clinical reminders as appropriate/due per the Electronic Medical Record (EMR). Administer immunizations per Unlicensed Assistive Personnel guidance. Service as clerical support for assigned PACT as well as all members of primary care. Process all primary care visitors and telephone calls to the clinic regarding availability of appointments for care and/or procedures, provide assistance with regard to changes in scheduling appointments and/or procedures. Responsible to obtain correct demographic patient information on all patients. Employee initiates patient appointments through EHR by utilizing the scheduling menus in order to establish proper clinical appointments. Communicate effectively with patients, families, interdisciplinary team members and other staff personnel. Act in a professional manner. Display courteous verbal and non-verbal communication. Communicate tactfully and effectively, both orally and in writing, in order to meet unit objectives. May include preparing reports in various formats and/or presenting information to the provider or supervisor. Utilize population, cultural, age-specific communication techniques to provide effective communication with patients and visitors. Scheduling Management Duties: Patient scheduling duties will utilize the authorized, official outpatient scheduling package in EHR software. All outpatient appointment requests must be acted upon within 7 calendar days from the date of the request. Veterans must have an appointment scheduled within 30 days of the desired date. Scheduling comments are to include information that may have been provided by the patient or is pertinent to the type of appointment or the scheduling of the appointment. When the patient is to be seen in multiple clinics, every effort is made to coordinate appointments to enable the veteran to reduce the number of visits to the treating facility. Assist in processing patients by recording chief complaints, brief history and other data describing the patient. Utilize VISTA and EHR to request supplies, services, read incoming publications, regulations, directives, schedule patient appointments, laboratory procedures, and radiology procedures. Enter encounter forms, transcribes orders, and other workload duties as appropriate. Daily review of active/pending consults, electronic wait list, recalls list and audio care/telephone messages and appropriate scheduling of those patients. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education or Training: One year above high school with courses related to the NA occupation. Completion of an intensive, specialized, occupation-related course of study of less than one year as a nursing assistant may also meet in full the experience requirements for GS-3. English Language Proficiency: NAs must be proficient in spoken and written English in accordance with VA Handbook 5005, part II, chapter 3, section A, paragraph 3. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-3: None beyond the basic requirements. GS-4: Experience: One year of specialized experience as a NA or experience in another medical field (licensed practical nurse, health technician, hospital corpsman, etc.), performing technical and nursing skills requiring knowledge of the human physical and emotional states, therapeutic communication, and technical skills required for basic and intermediate treatments; OR Education: Successful completion of 2 years above high school in an accredited institution with courses related to nursing care, health care or in a field of study appropriate to the specialization of the position, such as education in a program for psychology, psychiatric, or operating room technicians. AND In addition to the above experience/education, the candidate must demonstrate the following KSAs: Ability to provide a range of patient/resident care and contribute to the completion of the nurse care plan and recovery of patients/residents. Ability to observe patients/residents' or resident's physical or emotional status and refer deviations and/or problems to supervisor in a timely manner. Ability to properly use equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, blood glucose monitoring machine, specimen collection, etc. Ability to communicate effectively with patients/residents, their families and other health professionals. GS-5: Experience: One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment; OR Education: Successful completion of a 4-year course of study above high school leading to a bachelor's degree that included 24 semester hours of courses related to health care or possession of a bachelor's degree. AND In addition to the above experience/education, the candidate must demonstrate the following KSAs: Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse. Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new [NAs] by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance.
