Plans, implements and evaluates community health promotion activities in the spirit of Mercy Health mission and values. Teaches diverse community audiences about a variety of health related topics. Coordinates community program such as Neighborhood Health Watch (NHW), Faith Community Nursing (FCN) and Hispanic Health Programs. Responsible for site activities including health screenings and health education. Identifies appropriate community partners such as community centers, churches, restaurants, businesses as appropriate to the program. Responsible for data collection and report preparation about activities. Recruits, organizes and evaluates instructors, support staff, community members needed to accomplish health promotion activities. Markets and advertises programs to appropriate audiences.

Essential Functions

  • Provides health screenings such as blood pressure, body fat, bone density, blood sugar, and cholesterol at sites appropriate to program.

  • Identifies community partners to collaborate with programs and screenings.Provides support so that health related activities can be in a variety of locations. Support includes referral to resources, screenings and health education materials and speakers.

  • Refers clients to appropriate medical/social agencies including prescription drug assistance information. Provides appropriate follow up to clients with critical screening levels. Applies the nursing process in providing client care and in establishing a plan of care. Maintains client records in accordance with agency and funding source requirements.

  • Collaborates with team partner(s), specifically the site contacts, staff of the MHY Physician Practices, and community members to integrate care delivery with the goal of improved outcomes.

  • Markets and advertises programs and other health promotion activities to appropriate audiences. Collaborates with other Mercy Health departments and physicians, and community agencies and businesses in the provision of health promotion activities. Evaluates the effectiveness of health promotion activites.

  • Plans, implements and evaluates health promotion activities according to legal, ethical and professional standards. Utilizes effective teaching methods to teach diverse community audiences about a variety of health related topics. Utilizes needs assessment to determine the health education needs of the community and determines measureable outcomes (goals and objectives) for programs.

  • Recruits, organizes and evaluates the support staff, instructors and resources needed to accomplish health promotion activities. Develops and implements the quality improvement plan for Mercy Health promotion activities.

  • Represents Mercy Health in a positive manner at community health fairs, community screenings and other wellness programs and events. Participates in health promotion related committees, task forces and meetings within the organization and community.

  • Provides input to the Manager of Community Health Education for the fiscal operation of health promotion activities and effectively manages budget for health promotion programs. Assists with writing grants for health promotion programs and tracks budgets and statistics in order to provide appropriate reports to funding agencies.

Minimum Requirements

Required Minimum- Bachelor Degree Registered Nurse

Licensure/Certifications- OH RN License

Required: CPR instructor or within 6 months of hire

Preferred: Faith Community Nursing Certificate

Minimum Qualifications

1 - 2 years experience in acute care required. Proven experience interacting with diverse community groups is required.

Knowledge and experience with the health issues of and community resources available for the minority community within Mercy Health service area is required. Competent in providing health screenings such as blood pressue, body fat, bone density, blood sugar and cholesterol.

Experience in planning, implementing and evaluating health education programs for the community is a plus. Experience working with churches is a plus. Competent in use of computer for Microsoft Office programs (Word, Powerpoint, and Excel). Effectively can operate audiovisual equipment for program presentations.
